Starting Monday, September 14, students or their parents will be required to complete and submit the Daily Coronavirus Student Screening Form in PowerSchool BEFORE students leave home each morning. View instructions: English | Español.
- The Daily Coronavirus Student Screening Form must be completed every day that a student reports to school for in-person learning. Forms should not be submitted during students’ remote learning weeks.
- Access the Screening Form by logging into the PowerSchool website at https://powerschool.socsd.org/public. The website is mobile-responsive which allows users to access the form via their cell phones, tablets, etc. The form is not available in the PowerSchool mobile app at this time. The form will be live in PowerSchool this weekend.
- If the answer to any of the screening questions is “Yes,” students must remain at home AND parents should call their child’s school nurse.
Compliance is a critical component of our District’s health and safety protocols for reopening. We are relying on our families for their partnership in helping to keep our school district community healthy and safe.
