Annual Physical
Examination Form: This
form should be filled out the the child's pediatrician and
returned to the school nurse. You will need to print out and
complete one form for each child enrolled in the district.
Annual Medical History
Form: This form should be
filled out the the child's parent or guardian and returned to the
school nurse. You will need to print out and complete one form
for each child enrolled in the district.
Annual Dental
Examination Form: This form should be filled out the
the child's dentist and returned to the school nurse. You
will need to print out and complete one form for each child
enrolled in the district.
Physical Update
form for TZHS returning athletes:
This must be filled out by a parent or guardian each year and
returned to the nurse in order for a student to participate on a
team. If this is the first year trying out for a team, the
student will need a physical from a doctor, not this form.
Emergency
Contact Information:
Update your emergency contact information any time with this
form. It should be printed out and filled out by the parent or
guardian, and then returned to the school's main office.
