Forms/Materials for Parents

Adobe Acrobat Reader is required to view forms posted below in Adobe Portable Document Format (PDF) and may be downloaded free from the Adobe website. Unable to access the forms? Please contact the Transportation Office via email ([email protected]) or by calling (845) 680-1662 for assistance.

Please submit completed forms to the Transportation Office.

Alternate Site Transportation

To request transportation to or from a location other than your child’s registered address, an Alternate Site Request Form, posted under “Forms/Materials for Parents” above, must be completed and submitted to the Transportation Office by July 1 in order to have alternate transportation in place for the start of school. Requests received after the deadline will not take effect until after the first two weeks of school. Please allow 72 hours for requests made after the first two weeks of the school year to take effect.

Bus Designee

(WOS only)
William O. Schaefer Elementary School parents: If an adult other than a parent will receive your child at the bus stop, you must submit a signed Bus Designee Form, posted under “Forms/Materials for Parents” above, to the Transportation Office by August 1.

Bus Route Safety

Bus routes are created with student safety as a top priority. All questions regarding bus routes should be directed to the Transportation Coordinator. Bus companies will not make route changes. To request that the safety of a bus stop be re-evaluated, please submit the Bus Stop Safety Evaluation Request Form, posted under “Forms/Materials for Parents” above, to the Transportation Office.

Forgotten Items and General Questions

The South Orangetown Central School District does not own a bus fleet; it contracts with Student Bus, Peter Brega and Rockland BOCES. Forgotten items and general questions regarding the daily bus run may be directed to the bus companies:

Non-Public School Transportation

To request transportation to a non-public school, a Request for Transportation to Private School Form, posted under “Forms/Materials for Parents” above, must be submitted to the Registration Office by April 1 for the next school year. The Transportation Department does not provide transportation for any students on the days listed below for the 2018-19 school year.
